29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  A)    Mike Krueger DBA All Together Roofing, conducting work at 10429 White Oak Drive, Perrysburg, OH 43551: On or about September 15, 2016, the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  A)    Mike Krueger DBA All Together Roofing, conducting work at 10429 White Oak Drive, Perrysburg, OH 43551: On or about September 15, 2016, the employer did not ensure that employees were protected from head injury hazards by protective hard hats while working below employees who were replacing 8/12 pitch roof of the two story residential building. Employees were exposed to potential impact injuries caused by falling materials and/or equipment.    Mike Krueger was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.100(a)), which was contained in OSHA Inspection # 97330, Citation number 1, Item number 2, and was affirmed as a final order on December 8, 2011, with respect to a workplace located at 474 Parker Avenue, Toledo, OH 43605.

Employees were exposed to approximately 20 feet fall hazard from the roof to the ground below.    Mike Krueger was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.100(a)), which was contained in OSHA Inspection # 97330, Citation number 2, Item number 1, and was affirmed as a final order on December 8, 2011, with respect to a workplace located at 474 Parker Avenue, Toledo, OH 43605.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  A)    Mike Krueger DBA All Together Roofing, conducting work at 10429 White Oak Drive, Perrysburg, OH 43551: On or about September 15, 2016, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while replacing 8/12 pitch roof of the two story residential building. Employees did not use fall protection and were exposed to approximately 20 feet fall hazard from the roof to the ground below.    Mike Krueger was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.100(a)), which was contained in OSHA Inspection # 97330, Citation number 1, Item number 5, and was affirmed as a final order on December 8, 2011, with respect to a workplace located at 474 Parker Avenue, Toledo, OH 43605.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A)    Mike Krueger DBA All Together Roofing, conducting work at 10429 White Oak Drive, Perrysburg, OH 43551: On or about September 15, 2016, the employer did not ensure that extension ladders, used to access the roof of the two story residential building, were extended at least 3 feet above the landing surface. The employees were exposed to approximately 10 feet fall hazards when using the ladders to access the working level in order to conduct roofing activities.    Mike Krueger was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.100(a)), which was contained in OSHA Inspection # 97330, Citation number 2, Item number 2, and was affirmed as a final order on December 8, 2011, with respect to a workplace located at 474 Parker Avenue, Toledo, OH 43605.
