29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) On December 10, 2016, the employer did not ensure that energy isolating procedures for shutdown were performed prior to unfolding the overlapped conveyor belt #1 from the Chop Line. Employees performing this task in an energized condition were exposed to amputation hazards at the machines rotating rollers.
