29 CFR 1910.1200(e)(1): Employer had not developed, implemented or maintained at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met:    (a)  Innovations Manufacturing Inc, at 4555 Grape Street, Denver, CO: On and preceding 12/29/16, the employer had not developed, implemented or maintained at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f) and (g) of this section for labels and other forms of warning, and safety data sheets will be met.  Employees used saws and punch presses to fabricate vinyl and aluminum products.  The employer had not developed a written hazard communication program with regards to labeling and safety data sheets related to hazardous chemicals in their work area, including but not limited to the following:    1.  Polyvinyl chloride dust;  2.  Aluminum dust;  3.  Relton Corp Stick-Kut (containing paraffin wax);  4.  CRL Silicone Sealant; and  5.  Klean Strip Acetone.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, where employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response), or equivalently, a dose of fifty percent:    (a)  Innovations Manufacturing Inc, at 4555 Grape Street, Denver, CO: On 1/11/17, the employer did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, where employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response), or equivalently, a dose of fifty percent.  On 1/11/17 one employee operating a Sampson Machine Company double miter saw to cut aluminum and a Ryobi miter saw to cut vinyl was exposed to noise at a dose in excess of the fifty percent dose Action Level.  The employee was exposed to noise at a dose of 164.9%.  This is 3.30 times the Action Level.  The employer had not implemented a hearing conservation program.  This condition potentially exposed the employee to a hazardous noise dose.

Employees were exposed to respiratory hazards, flying object hazards, noise, and chemical hazards, including but not limited to the following:  1.  Exposure to Polyvinyl chloride dust when cutting vinyl using the Ryobi miter saws; 2.  Exposure to aluminum dust when cutting extruded aluminum using the Sampson Machine Company double miter saw or the Atech chop saw ; 3.  Exposure to paraffin wax fume when using Relton Corp Stick-Kut (containing paraffin wax) on the Atech chop saw; 4.  Exposure to CRL Silicone Sealant;  5.  Exposure to acetone when using Klean Strip Acetone; and 6.  Exposure to noise when operating miters saws, the chops saw, or the punch presses.
