29 CFR 1910.212(a)(2): General requirements for machine guards: guards were not affixed to the machine where possible and secured elsewhere if for any reason attached to the machine was not possible: a) Innovations Manufacturing Inc. at 4555 Grape Street, Denver, CO: on and preceding April 7, 2017 , the employer did not ensure that the clamps used to secure the material being cut was in place for the Atech Pegasus 14" Miter saw. The saw was used to cut aluminum stock. An employee experienced and finger amputation while using this saw without the clamps. This condition exposed employees to an amputation hazard.
