29 CFR 1910.213(c)(1): A circular hand-fed ripsaw was not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut. (a) Innovations Manufacturing, Inc., at 4555 Grape Street, Denver, CO 80216: On or before March 31, 2021, the employer did not protect employees from injury, in that an operator's hand made contact with the saw blade on the 10-inch Tilting Arbor Saw while he was cutting wood used for packaging, thus causing serious laceration injuries. An automatically adjusting hood which completely enclosed the portion of the saw above the table and above the material being cut was not installed. This condition exposed employees to laceration and amputation hazards. (b) Innovations Manufacturing, Inc., at 4555 Grape Street, Denver, CO 80216: On or before April 7, 2021, the employer did not ensure that an automatically adjusting hood which completely enclosed the portion of the saw above the table and above the material being cut was installed on the 10-inch Tilting Arbor Saw. Employees operated the saw to cut wood used in packaging. This condition exposed employees to laceration and amputation hazards. See Pictures below of required safety devices.

29 CFR 1910.213(c)(2): Hand-fed circular ripsaw was not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator: (a) Innovations Manufacturing, Inc., at 4555 Grape Street, Denver, CO 80216: On or before April 7, 2021, the employer did not ensure that a spreader was installed on the 10-inch Tilting Arbor Saw. Employees used the saw to cut wood used in packaging. This condition exposed employees to struck-by, laceration, and amputation hazards. Note: The provision of a spreader in connection with grooving, dadoing, or rabbeting is not required. On the completion of such operations, the spreader shall be immediately replaced.
