29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On and before August 25, 2022, the employer did not ensure that the point of operation on the Vinyl Glazing Bead Punch was properly guarded to prevent employees from having any parts of their body in the danger zone during the operating cycle. Operators operated the Vinyl Glazing Bead Punch with the front facing guard removed. The front facing guard was removed to clear punched material out of the point of operation. This condition exposed employees to lacerations, fractures, and amputations. (b) On August 25, 2022, the employer did not ensure that the point of operation on the Vinyl Glazing Bead Punch was properly guarded to prevent employees from having any parts of their body in the danger zone during the operating cycle, in that the operator sustained an amputation injury. This condition exposed employees to lacerations, fractures, and amputations.
