29 CFR 1910.36(b)(1): At least two exits routes were not available to permit prompt evacuation of employees during an emergency:    a) Innovations Manufacturing Inc. at 4555 Grape Street, Denver, CO:  On and preceding December 29th 2016, the employer did not ensure at least two exits routes were available to permit prompt evacuation of employees during an emergency.  The saw room was constructed of two metal framed walls covered in 1/2" thick plywood and two poured concrete walls with the space measuring approximately 36'x 61'.  There was a single exit door. The employees used saws and punch presses to fabricate aluminum and vinyl products.  The employer did not ensure that at least two egress routes were available for employees to exit the saw room.  This condition exposed employees to a fire hazard.

29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:    a) Innovations Manufacturing Inc. at 4555 Grape Street, Denver, CO:  On and preceding December 29th 2016, the employer did not ensure point of operation of machinery was guarded to prevent employees from having any part of their body in the danger zones during operating cycles.  One employee operated the Sampson Machine Co. model MN15012 Double Miter saw to cut vinyl. The employees used saws to fabricate vinyl products. This condition exposed employees to an amputation hazard.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power presses:  a) Innovations Manufacturing Inc. at 4555 Grape Street, Denver, CO:  On and preceding December 29th 2016, the employer did not ensure point of operation of machinery was guarded to prevent employees from having any part of their body in the danger zones during operating cycles.  Point of operation guards were not installed on the 3 ton single action mechanical punch press. Employees used punch presses to fabricate aluminum products. This condition exposed employees to an amputation hazard; and  b) Innovations Manufacturing Inc. at 4555 Grape Street, Denver, CO:  On and preceding December 29th 2016, the employer did not ensure point of operation of machinery was guarded to prevent employees from having any part of their body in the danger zones during operating cycles.  Point of operation guards were not installed on the 5 ton continuous action mechanical punch press. Employees used punch presses to fabricate aluminum products. This condition exposed employees to an amputation hazard.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Innovations Manufacturing Inc. at 4555 Grape Street, Denver, CO:  On and preceding December 29th 2016, the employer did not ensure listed or labeled electrical equipment was used or installed in accordance with instructions included in the listing or labeling.  The saw room was equipped with three single miter saws, one chop saw, one double miter saw, shop lighting and other portable equipment.  This equipment was powered by flexible cords that were in series with power strips and additional flexible extension cords. The flexible cords and power strips were daisy chained.  The employees used saws and punch presses to fabricate aluminum and vinyl products. This condition exposed the employees to a fire hazard.

The double miter saw, chop saw, punch press and other equipment throughout the saw room was powered by flexible power cords and power strips due to an insufficient number of outlets. This condition exposed employees to fire hazards.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors:    a) Innovations Manufacturing Inc. at 4555 Grape Street, Denver, CO:  On and preceding December 29th 2016, the employer did not ensure flexible power cords were not run through holes in walls, ceilings, or floors.  The Atech Pegasus chop saw was plugged into a flexible extension cord which was run through a hole in the wall in the saw room and routed to an electrical panel outside of the saw room.  This condition exposed the employees to electrical an hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a) Innovations Manufacturing Inc. at 4555 Grape Street, Denver, CO:  On or before December 29th 2016, the employer did not visually inspect portable cord and plug-connected equipment and flexible cord sets before use for external defects and for evidence of possible internal damage.  The flexible power cord providing power to the 5 ton continuous operation punch press was damaged.  The insulation was damaged and the conductor was exposed.  This condition exposed the employees to an electrical hazard.
