29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a. On or about January 24, 2017 employees using portable electrical hand tools, powered by portable gasoline driven generators, were exposed to electrical shock hazards because the path to ground on two temporary flexible extension cords, and one portable air compressor were not permanent and continuous, because the ground pin had been removed or broken off.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on January 24, 2017 when employees are working on installing floor trusses on top of the first floor exterior walls of a multi-unit residential housing complex to frame the second floor at Arbor Crossing @ Buck Lake, Tallahassee, FL without fall protection and were exposed to a fall in excess of 9 feet to the ground below. Garcia Building Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1005607, citation number 1, item number 1 and was affirmed as a final order on January 20, 2015, with respect to a workplace located at 10247 Hwy 92, Woodstock, GA 30188.
