29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. On or about January 25, 2017, roof area, employees tearing off clay tiles on a 4:12 pitch roof were not protected from a 13 feet fall hazard by the use of a fall protection system.  The employer fails to assure effective protection against falls as required by the standard, as demonstrated on January 25, 2017, when employees tearing off and old roof system on a 4:12 pitch roof located at 541 John Anderson Drive, Ormond Beach, FL are exposed to a 13 feet fall hazard above the ground in that they did not have a fall protection system installed.  King Construction USA, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 950845, citation number 02, item number 001 and was affirmed as a final order on January 2, 2014, with respect to a workplace located at 5200 North Orange Blossom, Orlando, FL 32810.  King Construction USA, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 915728, citation number 01, item number 001 and was affirmed as a final order on August 2, 2013, with respect to a workplace located at Garden Park, Fern Park, FL 32730.
