29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work on had the strength and structural integrity to support employees safely:    (a) On or about April 15, 2019, the wooden roof structure where employees walked and worked to prepare the roof for the replacement of the concrete roof tiles, roofing felt paper and rotted wood was not inspected to determine areas that were deteriorated, exposing employees to a 17 feet, 2 inch fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) On or about April 16, 2019, fall protection training provided to employees was inadequate, exposing employees to fall hazards in that, employees disconnected and were allowed to disconnect from the personal fall arrest system while utilizing a backpack blower on a roof and connect to a ring on the front of the harness not intended for a fall arrest system.    (b) On or about April 16, 2019, fall protection training provided to employees was inadequate, exposing employees to fall hazards in that, employees did not know at what height fall protection equipment was required by OSHA.    (c) On or about April 16, 2019, fall protection training provided to employees was inadequate, exposing employees to fall hazards, in that, more than one employee connected and was allowed to connect to a single person anchorage point.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a) On or about April 15 2019, an employee was allowed to disconnect from the personal fall arrest system and connect the lanyard to a ring on the front of the harness while using a backpack blower to blow roof debris from a 6/12 sloped roof, exposing the employee to a 17 feet, 2 inch fall hazard.    The King Construction USA, Inc. was previously cited for a Repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 1205851, citation number 01, item number 001 and was affirmed as a final order on April 17, 2017, with respect to a workplace located at 541 John Anderson Drive, Ormond Beach, FL 32176.
