29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  Mike Krueger dba Altogether Roofing, LLC, conducting work at 9520 Jerusalem Road, Curtice, Ohio:  On or about December 19, 2017 the employer did not provide abatement verification for the following citation items:  Citation 1-1a, 1-1, and 1-2.  The citation items became final order on October 19, 2017, with an abatement due date of October 19, 2017.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  A) Mike Krueger DBA Altogether Roofing LLC, conducting work at 9520 Jerusalem Road, Curtice, OH, 43412: On or about May 3, 2017 the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.   Mike Krueger was previously cited for a violation of this occupational safety and health standard (1926.20(b)(1)), which was contained in OSHA Inspection # 1177422, Citation number 1, Item number 1, with respect to a workplace located at 10429 White Oak Drive, Perrysburg, OH, 43551, and was affirmed as a final order on March 3, 2017.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  A)  Mike Krueger DBA Altogether Roofing LLC, conducting work at 9520 Jerusalem Road, Curtice, OH, 43412: On or about May 3, 2017  the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while replacing a 6/12 pitch roof of the two story residential building. Employees did not use fall protection and were exposed to approximately 20 feet fall hazard from the roof to the ground below.  Mike Krueger was previously cited for a violation of this occupational safety and health standard (1926.503(a)(1)), which was contained in OSHA Inspection # 1177422, Citation number 2, Item number 2b, with respect to a workplace located at 10429 White Oak Drive, Perrysburg, OH, 43551, and was affirmed as a final order on March 3, 2017.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  A) Mike Krueger DBA Altogether Roofing LLC, conducting work at 9520 Jerusalem Road, Curtice, OH 43412: On or about May 3, 2017, the employer did not ensure that employees were protected from fall hazards while replacing a 6/12 pitch roof of the two story residential building. Employees were exposed to approximately 20 feet fall hazard from the roof to the ground below.  Mike Krueger was previously cited for a violation of this occupational safety and health standard (1926.501(b)(13)), which was contained in OSHA Inspection # 1177422, Citation number 2, Item number 2a, with respect to a workplace located at 10429 White Oak Drive, Perrysburg, OH, 43551, and was affirmed as a final order on March 3, 2017.
