McCarthy Way, Perrysburg, Ohio:  On or about December 19, 2017 the employer did not provide abatement verification for the following citation items:  1-1a, 1-1b, and 1-2.  The citation items became final order on October 31, 2017, with an abatement due date of October 31, 2017.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with this part.  A) Casey Bortles worksite located at 25261 John F. McCarthy Way, Perrysburg, Ohio:  The employer failed to ensure an accident prevention program was in place to aid the employer and his employees in the recognition and avoidance of hazards associated with residential roofing operations (i.e.; fall protection, and use of personal protective equipment).    Casey Bortles was previously cited for a violation of this occupational safety and health standard 1926.20(b)(1), which was contained in OSHA inspection 1066332, citation number 1, item number 1a and was affirmed as a final order on July 14, 2015 with respect to a workplace located at Perrysburg, Ohio.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Casey Bortles on May 9, 2017 at the address of 25261 John McCarthy Way, Perrysburg, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations.  Casey Bortles was previously cited for a violation of this occupational safety and health standard 1926.20(b)(1), which was contained in OSHA inspection 1066332, citation number 1, item number 1b and was affirmed as a final order on July 14, 2015 with respect to a workplace located at Perrysburg, Ohio.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.   A. Casey Bortles worksite located at 25261 John F. McCarthy Way, Perrysburg, Ohio: On or about May 9, 2017, the employer did not ensure employees were protected from falls while performing roofing activities on a two story residential structure. The employees were exposed to a potential fall distance of approximately 20 feet.   Casey Bortles was previously cited for a violation of this occupational safety and health standard 1926.501, which was contained in OSHA inspection 1066332, citation number 1, item number 2 and was affirmed as a final order on July 14, 2015, with respect to a workplace located at Perrysburg, Ohio.
