29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. Note: The following appendices to subpart M of this part serve as non-mandatory guidelines to assist employers in complying with the appropriate requirements of subpart M of this part. [59 FR 40738, Aug. 9, 1994; 60 FR 5131, Jan. 26, 1995] a) Centre Square Fire Station, Worksite - on or about May 18, 2017, employees are performing frame carpentry work on a commercial construction project, approximately 18 feet above grade, without utilizing any means of fall protection. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Centre Square Fire Station, Worksite - on or about May 18, 2017, employees are performing frame carpentry work on a commercial construction project, approximately 18 feet above grade, while accessing the upper working surfaces by climbing the exterior framing. b) Centre Square Fire Station, Worksite - on or about May 18, 2017, employees are performing frame carpentry work on a commercial construction project, while accessing the upper working surface by climbing the guardrails of a scissor lift and onto lower chord of the trusswork, a difference of approximately 42 inches.
