1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail systems; Safety net systems; or Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: a) On or about May 24, 2017 employees performed cleaning tasks and/or plate changes on deck 1 on the Comexi printing press while standing and/or laying on a narrow platform and were not protected from a fall hazard of over 4 feet. b) On or about May 24, 2017 employees performed cleaning tasks and/or plate changes on deck 8 on the Comexi printing press while standing and/or laying on a narrow platform and were not protected from a fall hazard of over 4 feet.

1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment; communicate selection decisions to each affected employee; and, select PPE that properly fits each affected employee: On or about May 24, 2017 the hazard assessment performed did not identify burns and/or skin irritation caused by Cell Clean to exposed skin. Employees using the chemical for cleaning tasks were not instructed to utilize sleeves and/or longer gloves to protect their arms.
