29 CFR 1910.184(c)(6):Slings shall be securely attached to their loads. On or about September 14, 2022, at an establishment located at 1327 Glory Road in Green Bay, Wisconsin, in the Production Area: employees using All-Lift Systems LBB1-940 X 7 Nylon Load Balancer Baskets, to lift rolls of substrate weighing up to 1,200 pounds, were exposed to crushed-by and struck-by hazards created when the eyes of the basket were not seated in the bowl of the single-point hook preventing the safety latch from closing and securing the load.
