(a)  The employer did not develop specific procedural steps on the purpose of the procedure, specific procedural steps to de-energization, specific procedural steps for application of energy isolating device, specific procedural steps for verification of de-energization when performing tasks such as but not limited to: cleaning and set-up activities on the Fusion C, Fusion C+, Comexi FX2108, ProVision, Chadwick, Comexi F2MP, Fusion C3, Fusion Max,  and Fischer & Krecke.  (b)  The Chadwick energy control procedure developed did not address the natural gas and thermal energy sources exposing employees to burn hazards.

29 CFR  1910.147(d):  The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a)  In March 2021 and thereafter, employees were exposed to machine hazards associated with moving parts when they engaged in change over activities on the printing press equipment.  The employer did not ensure that employees implemented energy control application steps prior to allowing them to remove and change the anilox rollers and plates.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  (b)  In March 2021 and thereafter, employees were exposed to machine hazards associated with moving parts when they engaged in cleaning activities on the printing press equipment.  The employer did not ensure that employees implemented energy control application steps or alternative protective measures prior to allowing them to clean the anilox rollers and plates.  The machine was not deenergized prior to employee engaging in the servicing/maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
