29 CFR  1910.106(e)(2)(ii)(b)(2): The quantity of liquid that may be located outside of an inside storage room or storage cabinet in a building or in any one fire area of a building shall not exceed: 120 gallons of Category 2, 3, or 4 flammable liquids in containers.  In the Press Room near the east wall , the employer did not ensure that Category 2 and 3 flammable liquids stored outside of an inside storage room or storage cabinet in the building or in any one fire area of the building did not exceed 120 gallons, exposing employees to fire and burn hazards in the following instances:  a) One steel 55-gallon drum containing approximately 27 gallons of Acetone (Category 2 flammable liquid); b) Two steel 55-gallon drums containing approximately 110 gallons of N-Propyl Acetate (Category 2 flammable liquid); c) One steel 55-gallon drum containing approximately 55 gallons of Barsol PNP (Category 3 flammable liquid); d) Two steel 55-gallon drums containing approximately 110 gallons of N-Propyl Alcohol (Category 3 flammable liquid).

29 CFR  1910.106(e)(6)(i): "General." Adequate precautions shall be taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat.  The employer did not ensure that containers used to transfer flammable liquids were electrically interconnected to prevent the accumulation of static electricity, exposing employees to fire and burn hazards in the following instances:  a) At the Fischer & Krenke Press, employees transferring IPA 99 (Category 2 flammable liquid) from IBCs into metal containers without the containers being grounded and bonded; b) At the Comexi FX2108, employees transferring Barsol A-2154 (Category 2 flammable liquids) from IBCs into metal containers without the containers being bonded; c) At the Comexi FX2108 Press, employees transferring IPA 99 (Category 2 flammable liquid) from metal containers into metal containers without the containers being bonded; d) At the Fusion C Press, employees transferring IPA 99 (Category 2 flammable liquid) from a metal container to another container without the containers being grounded and bonded; e) At the Fusion Max Press, employees transferring IPA 99 and Barsol A-2154 (Category 2 flammable liquids) from metal containers into metal containers without the containers being bonded.

29 CFR  1910.106(e)(9)(iv):"Clear zone." Ground area around buildings and unit operating areas shall be kept free of weeds, trash, or other unnecessary combustible materials.  At the Provision Press, a stainless steel IBC container holding Barsol A-2154 (Category 2 flammable liquid) was stored unnecessarily on top of five wood pallets, a combustible material, exposing employees to fire and burn hazards.

29 CFR  1910.1200(e)(1):Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met.  The employer did not ensure that their Hazard Communication Program included all of the elements as required in the Hazard Communication Standard, exposing employees to health and fire hazards. Elements that were not included in the employer's Hazard Communication Program included:  a) Explanation of alternative labeling methods for chemicals used in the facility, making it difficult to identify the chemical and the hazards present; b) Informing employees of the hazards in non-routine tasks; c) Methods to provide the other employers on-site access to SDSs, precautionary measures, and labeling system used.

The maximum allowable size for Category 2 flammable liquids in glass or approved plastic containers was 1 quart.  At the Provision Press, the employer did not ensure that a container containing IPA 99 (Category 2 flammable liquid) was stored in a container with a maximum allowable size of one quart, exposing an employee to fire and burn hazards.
