This paragraph also requires the employer to provide the basic information on respirators in Appendix D of this section to employees who wear respirators when not required by this section or by the employer to do so. a) On or about June 2, 2024, at the establishment located at 1327 Glory Road, Green Bay, Wisconsin; the employer did not ensure that each employee that voluntarily used a filtering facepiece respirator was trained on the information contained in Appendix D of 29 CFR 1910.134, exposing an employee to respiratory hazards.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a) On or about and at times prior to June 10, 2024, at the establishment located at 1327 Glory Road, Green Bay, Wisconsin; the employer did not ensure that employees using the ammonium hydroxide solution at the presses were wearing the correct PPE (face shield or goggles and chemical-resistant gloves) as required by the hazard assessment, exposing employees to chemical hazards associated with ammonium hydroxide. Legacy Flexo was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.132(a), which was contained in OSHA inspection number 1521408, citation number 1, item number 2, and was affirmed as a final order on June 17, 2021, with respect to the establishment located 1327 Glory Road, Green Bay, WI 54304.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Legacy Flexo - 1327 Glory Road, Green Bay, Wisconsin. On or about June 10, 2024, the employer did not provide the required injury and illness description on the OSHA Form 300 or equivalent for the calendar year 2024. a ) On or about May 12, 2024, while a Supervisor was troubleshooting a press, the employee sustained a cut to the right knuckle while troubleshooting a press (Case log Number 2), which resulted in an other recordable case. The Employer's OSHA 300 Form or equivalent for the calendar year 2024 did not contain a complete description of the object/substance that directly injured or made the person ill. b ) On or about May 16, 2024, while a Press Operator was working with chemicals during cleaning, the employee sustained an eye injury following exposure to ink while cleaning (Case log Number 3), which resulted in an other recordable case. The Employer's OSHA 300 Form or equivalent for the calendar year 2024 did not contain a complete description of the injury or illness and parts of the body affected. c ) On or about June 2, 2024, while a Press Operator was working with chemicals, the employee sustained a throat injury following exposure to ammonia (Case log Number 4), which resulted in an other recordable case. The Employer's OSHA 300 Form or equivalent for the calendar year 2024 did not contain a complete description of the injury or illness and parts of the body affected.
