29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    Throughout the production area, propane gas-powered sit-down counterbalanced forklifts: on or about June 6, 2017, each operator was not provided with workplace-specific and truck-specific formal and practical training in safe operation of the equipment prior to being permitted to use equipment for regular material handling tasks. Employees operating equipment and other employees working in the area were exposed to struck-by hazards likely to result in serious injury.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  Throughout the production area: On or about June 6, 2017, the employer failed to establish operator safety training for sit-down counterbalance forklifts consisting of practical training (demonstrations performed by the trainer and practical exercises performed by the trainee) to ensure through adequate evaluation that each operator is competent in safe operation of the equipment in the workplace. Employees were exposed to struck-by hazards likely to result in serious injury.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.179(b)(8): Only designated personnel shall be permitted to operate a crane covered by this section:    Northwest, EMH 5 Ton and EMH 20 Ton overhead bridge cranes - on or about June 6, 2017, The employer failed to adequately qualify personnel designated to operate cranes through adequate formal and practical training and written and practical examination in safe operation. Employees operating cranes for regular production load handling tasks (e.g. moving loaded racks of aluminum products) and other employees working in the area, were exposed to load shifting and falling struck-by and crushed-under hazards likely to result in serious injury.    (a) some employees were permitted to utilize equipment prior to being qualified through any formal and practical training in safe operation, and without successfully completing any written and practical examination; and,    (b) some employees were permitted to utilize equipment after receiving some formal training in safe operation, but prior to being qualified through any practical training, and without successfully completing any practical examination.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service.    Northwest, EMH 5 Ton overhead bridge crane no. 158270, Rockford 4-leg bridle synthetic web sling: on or about June 6, 2017, operators were exposed to hazards associated with shifting and falling loads when adequate nylon sling inspections were not conducted daily to identify damages requiring removal from service prior to use for handling loaded racks.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

There is no abatement documentation or certification required for this item.
