29 CFR 1910.179(e)(2)(i): A crane shall be provided with bumpers or other automatic means providing equivalent effect, unless the crane travels at a slow rate of speed and has a faster deceleration rate due to the use of sleeve bearings, or is not operated near the ends of bridge and trolley travel, or is restricted to a limited distance by the nature of the crane operation and there is no hazard of striking any object in this limited distance, or is used in similar operating conditions. The bumpers shall be capable of stopping the crane (not including the lifted load) at an average rate of deceleration not to exceed 3 ft/s/s when traveling in either direction at 20 percent of the rated load speed: Northwest, EMH 20 Ton overhead double girder top-running bridge crane, serial no. 147838: on or about May 8, 2017, bumpers, stops, and/or other equivalent means of reducing and stopping bridge travel near or at the end of the operational portion of the runway were not securely installed and aligned to prevent derailment of the bridge. Employees were regularly operating the crane for material handling tasks. There is no abatement certification or documentation required for this item.

29 CFR 1910.179(k)(2): Rated load test. Test loads shall not be more than 125 percent of the rated load unless otherwise recommended by the manufacturer. The test reports shall be placed on file where readily available to appointed personnel: Northwest, EMH 5 Ton (serial no. 158270) and EMH 20 Ton (147838) overhead double girder top-running bridge cranes: on or about June 20, 2017, rated load test reports were not available upon request to determine structural and operational integrity of the equipment to handle loads at the rated weight limit. Employees were operating cranes for regular production material handling tasks. There is no abatement certification or documentation required for this item.
