On or about June 13, 2019, the scaffold platform spanning the roof opening was not erected under the supervision of a competent person qualified in scaffold erection, which resulted in reduced stability of the scaffold platforms, a partially planked platform and employee exposure to falls of at least 31 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold.    On or about June 13, 2019, employees without fall protection were exposed to fall hazards of approximately 31 feet while working from a scaffold platform that was spanning an unprotected roof opening. On or about June 13, 2019, employees were required to work on unsecured scaffold planks that had been erected over a roof opening.  The employer did not ensure each employee involved in the erection of the scaffold was adequately trained to recognize hazards associated with scaffold erection.
