Employees were exposed to a 43-feet fall hazard.   b) The employer fails to ensure effective protection against falls as required by the standard, as demonstrated on June 13, 2017: when four employees  installing wood frame elements on roof trusses were not protected by guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure.
