29 CFR 1910.147(c)(6)(i): The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. Examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points,and rotating parts. Employees were exposed to amputation and crushed-by hazards. Employees working in the production area were exposed to unguarded rotating parts of equipment. Machine guarding was not provided for the roller assembly on the 202 Adjuster , known to the company as Winkler+Dunnebeir 202 Serial Number 11766, Tension Envelope Corp permanent machine number 576.
