29 CFR  1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected:   On or about May 13, 2021 and at times prior thereto, the employer did not perform periodic inspections between each authorized employees and the inspector to determine the energy control procedure was working effectively, exposing employees to struck-by, caught-in and amputation hazards.

29 CFR 1910.147(c)(7): At this establishment, the employer had not provided training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices was acquired by the employees.  In the production area, the employer did not provide adequate training to ensure obtained the knowledge, skills and familiarization with the lockout/tagout application and procedures when operating and servicing equipment, such as but not limited to the 02-40-527 WEB R-7, envelope machine, serial number 9731, exposing employees to the hazard of being caught in or struck by the equipment.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  In the production area, the employer did not perform periodic annual inspections to ensure the Lockout/Tagout Program is working effectively, such as but not limited to 02-40-527 WEB envelope folding machine. Reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.  Tension Envelope Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i), which was contained in OSHA inspection number 1248138, Citation Number 1, Item number 1, and was affirmed as a final order on October 17, 2017, with respect to a workplace located at 819 East 19th Street , Kansas City, Missouri 64108.
