29 CFR 1910.29(b)(1): The employer did not ensure that guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D11 of this section):  On or about October 28, 2022, and at times prior thereto, employees accessed the catwalks at envelope machines which were approximately 55-inches from the platform  to the ground level. Top rails installed were not installed at 42-inches, plus or minus 3-inches, above the walking-working surface on the following machines, exposing employees to fall hazards:   a) Envelope Machine Line 7; and b) Envelope Machine Line 8.

29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that  midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high:   On or about October 28, 2022, and at times prior thereto, employees were exposed to falls because midrails were not installed at least 21-inches high. Employees accessed the catwalks at a height of approximately 55-inches from the platform to the ground level on the following envelope machines:   a) Envelope Machine Line 7; and b) Envelope Machine Line 8.

29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  On or about October 28, 2022, and at times prior thereto, the employer's alternative lockout tag out procedures (Inch Safe) did not clearly and specifically outline the scope, purpose, authorization, rules, techniques to be utilized and the means to enforce compliance.   Tension Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(4)(ii)), which was contained in OSHA inspection number 1532978, citation number 1, item number 1, issued on July 26, 2021 and was affirmed as a final order on August 19, 2021, with respect to a workplace located at 5900 Tension Drive, Fort Worth, Texas 76112.
