29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: In the production area, the employer did not develop written lockout/tagout procedures describing how to lockout and control hazardous energy sources for affected and authorized employees for machines, such as the R-10 Folding Machine, which exposed employees to the hazard of being caught in between the chain and sprocket.
