29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. On or about August 15, 2017, on the east side of a roof, two employees installing asphalt shingles while using pneumatic nail guns did not wear eye protection, exposing the employees to the hazard of being struck in the eye or face by flying debris.

On or about August 15, 2017, on the east side of a 5/12 pitch roof of a new residential condominium, employees installing asphalt shingles were not protected from a 25 foot fall hazard by the use of a personal fall protection system.
