29 CFR 1915.87(f)(1): The employer did not provide an adequate number of basket stretchers, or the equivalent, readily accessible to where work is being performed on a vessel or vessel section.  Shipyard and/or floating dry dock: The facility's basket stretcher was not readily accessible being that it was located in a warehouse outside of the shipyard's gate. This would delay the ability to remove a worker from the vessel in the event of an emergency.

29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE.  Floating dry dock: Employees engaged in cleaning and rigging operations were not wearing personal floatation devices adjacent to unguarded edges. Employees were exposed to drowning hazards.

M/V Tamaraw, below the main deck, aft: On August 14, 2017, torch cutting was performed without fire watches constantly posted on and below the main aft deck. There was an opening in the deck which led to a storage area containing combustible materials, such as but not limited to cardboard boxes and contaminated oil filters. Employees were exposed to fire hazards.
