29 CFR 1915.172(d): A certification record of pressure vessel examinations and tests made in compliance with the requirements of 29 CFR 1915.172(a) and (b) was not maintained. F-2 Pier, adjacent to the dry dock: On April 09, 2018, a National BD, 200 gallon air compressor tank used to pressure test a fuel oil day tank onboard the CB Micronesian was not examined and certified to meet the standards of the American Society of Mechanical Engineers Boiler and Pressure Vessel Code, Section VIII, Rules for Construction of Unfired Pressure Vessels, 1963. Employees were exposed to struck-by hazards.
