29 CFR 1910.107(b)(5)(iv): Space within the spray booth on the downstream and upstream sides of filters were not protected with approved automatic sprinklers: Painting and blasting area. Employees are exposed to fire hazards from spraying flammable paints, coatings and epoxies inside a shipping container designated as a spray area without the use of automatic sprinklers or a fire suppression system.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) Painting and blasting area. Employees working in the painting and blaster area were exposed to chemical burns of the eyes and skin while using Intershield 300 Aluminum Part A to paint fabricated metal parts. There is no eyewash station or safety shower in the vicinity were this chemical is used. (b) Warehouse. Employees working in the warehouse were exposed to chemical burns of the eyes and skin while using Hurricane Fore Degreaser to clean and degrease parts. There is no eyewash station or safety shower in the vicinity were this chemical is used. (c) Warehouse. Employees were exposed to chemical burns of the eyes and skin while using a battery charging station located in the warehouse to charge lead-acid forklift batteries. There is no eyewash station or safety shower in the vicinity near the forklift charging station. (d) Warehouse. Maintenance Employees were exposed to chemical burns of the eyes and skin while adding water and other chemicals to lead-acid forklift batteries. There is no eyewash station or safety shower in the vicinity were water and chemicals are added to batteries. (e) Machine shop. Machine shop employees are exposed to chemical burns of the eyes and skin while using corrosive chemicals to clean recently fabricated metal parts. The eyewash station provided in the area is not suitable for use in that the dust covers have been removed allowing dust, oil and grease to accumulate on the spray heads.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: Metal fabrication area. Employees working in the metal fabrication area where exposed to chemical hazards from storing chemicals, such oils and lubricants in unlabeled water and Gatorade containers.

Machine shop employees were exposed to chemical hazards while using Skyco OSPHO Metal Treatment, but the employer did not keep a copy of the Safety Data Sheet (SDS) at the jobsite. (b) Machine Shop. Machine shop employees were exposed to chemical hazards while using CRC 2-26 Multi-Purpose Precision Lubricant, but the employer did not keep a copy of the Safety Data Sheet (SDS) at the jobsite; (c) Machine Shop. Machine shop employees were exposed to chemical hazards while using EIS Moto Finish BIGRED Alkyd Insulating Enamel Paint, but the employer did not keep a copy of the Safety Data Sheet (SDS) at the jobsite.
