29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    (a)  In the Top Building area, on or about August 31, 2017, a hearing conservation program was not implemented for an employee who was exposed to noise at 133% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 87.0 dBA during the 330 minute sampling period, resulting in a noise hazard.

29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    (a)  In Special Projects/Final Finish area, on or about August 30, 2017 and at times prior to, the Powermatic table saw was not equipped with a blade guard, resulting in an amputation hazard.
