29 CFR 1917.26(d)(1) as required by 1960.8(b): At least one Stokes basket stretcher, or its equivalent, was not available for each vessel being worked. 1. On or about September 12, 2017, at the Mainland Ranger III dock, the employer did not ensure that a stokes basket stretcher or its equivalent was available while the NPS Ranger III vessel was being loaded with materials, thereby exposing employees to a drowning hazard. 2. On or about September 12, 2017, at the Angelique LCM loading area, the employer did not ensure that a stokes basket stretcher or its equivalent was available while the Angelique LCM was being loaded with materials, thereby exposing employees to a drowning hazard.

29 CFR 1917.26(f)as required by 1960.8(b): A US Coast Guard approved 30 inch life ring, with at least 90 feet of line attached, was not available at readily accessible points at each waterside work area where the employees' work exposed them to the hazard of drowning. 1. On or about September 12, 2017, at the Mainland Ranger III dock, the employer did not ensure that a life ring located at the aft end of the vessel was not equipped with 90 feet attached of line while the vessel was being loaded with materials, thereby exposing employees to a drowning hazard. There was no line present at the life ring. 2. On or about September 12, 2017, at the Mainland Ranger III dock, the employer did not ensure that a life ring located at the forward end of the vessel was not equipped with 90 feet of line attached while the vessel was being loaded with materials, thereby exposing employees to a drowning hazard. There was a 70 foot throw bag of 3/8 inch polypropylene rope available but not secured to the life ring. 3. On or about September 12, 2017, at the Mainland Angelique LCM loading area, the employer did not ensure that a life ring was readily accessible near the vessel while the vessel was being loaded with equipment, thereby exposing employees to a drowning hazard.
