On or about September 14, 2017, at the Rock Harbor Activated sludge plant, the employer did not ensure that openings in the floor on top of the plant were protected by a guard rail system or covered there by exposing employees to a fall hazard into sewage in the activated sludge plant digester.

29 CFR 1910.305(g)(1)(iv)(A) as required by 1960.8(b): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: On or about September 13, 2017, at the Rock Harbor, Activated Sludge Plant Lab, the employer used flexible cable as a substitute for fixed wiring in that a flexible cord was used to power a two way outlet which provided power to an air conditioner and provided power to a radio there by exposing employees to an electrical hazard.
