Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5.    On or about September 4, 2017,  on Mott Island, employee was required to disconnect power to the main 480v transformer due to a connect failure on the load side of the transformer and the employer did not provide rubber insulating gloves that were periodically tested within the last six months there by exposing employee to electrical shock hazard. The rubber insulating gloves were last tested September 01, 2011.

One 4-leg bridle wire rope sling and associated hooks and shackles were not marked with the safe working load.

The abrasive wheel grinder was used to brush a binder bolt used in a transformer repair.

The travel lift was used to remove a boat from the water for maintenance.
