29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting face piece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): On or about September 13, 2017, the employer did not provide appropriate qualitative or quantitative fit tests to employees required to wear tight fitting full-faced air purifying respirators for work such as, but not limited to adding hydrated lime to waste water. The National Park Service was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1080937, Notice number 1, item number 1 and was affirmed as a final order on January 27, 2016, with respect to a workplace located at 5083 Foresta Road, El Portal, CA.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: On or about September 13, 2017, the employer did not ensure that tight fitting air purifying respirators required for work such as, but not limited to adding hydrated lime to waste water were stored to protect them from damage, contamination, and dust in that in the rock harbor maintenance shop a respirator was left sitting out on a work bench when not in use. The National Park service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.134(h)(2)(i), which was contained in OSHA inspection number 1139553, Notice number 1, item number 2b, and was affirmed as a final order on June 30, 2016, with respect to a workplace located at 612 East Reserve Street, Vancouver, WA.
