29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  On or about September 21, 2017, the employer failed to ensure that the ladder used to access the roof was at least 3 feet above the landing surface. Employees were performing roofing activates on a residential home located at 9410 Rockys Ct. Broadview Heights, OH 44147.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about September 21, 2017, employees were exposed to a fall hazard of approximately 19.7 feet while performing roofing activities on a residential home located at 9410 Rockys Ct. Broadview Heights, OH 44147. Employees did not utilize any form of fall protection during this activity.   CHANELL ROOFING & HOME IMPROVEMENT, LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1196440, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON OR ABOUT FEBRUARY 13, 2017, WITH RESPECT TO A WORK PLACE LOCATED AT 18719 DRAKE ROAD, STRONGSVILLE, OH 44136.     CHANELL ROOFING & HOME IMPROVEMENT, LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1099366, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON OR ABOUT FEBRUARY 22, 2016, WITH RESPECT TO A WORK PLACE LOCATED AT 2961 SPRING VALLEY PIKE, MIAMISBURG, OHIO.
