29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:   (a) Chanell Roofing & Home Improvement, LLC, on or about May 30th, 2019, at the location of 2375 and 2377 Manchester Ln., Avon, OH, 44011:  Employees were performing residential roofing activities without the benefit of an accident prevention program to ensure employees were protected from falling object hazards through head protection and fall hazards of 9 feet to the ground below through proper utilization and use of fall protection and ladders.

This condition was recently observed on or about May 30th, 2019, at the location of 2375 and 2377 Manchester Ln., Avon, OH, 44011, when employees were exposed to fall hazards of 9 feet, 5 inches while performing residential roofing work.   Chanell Roofing & Home Improvement, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(a)(1) which was contained in OSHA inspection number 1099366, citation number 1, item number 2, and was affirmed as a final order on 02/22/2016 with respect to a workplace located at 2961 Spring Valley Pike, Miamisburg, OH, 45342.

This condition was most recently observed on or about May 30th, 2019, at the location of 2375 and 2377 Manchester Ln., Avon, OH, when employees were exposed to fall hazards of 9 feet, 5 inches to the ground below.   Chanell Roofing & Home Improvement, LLC was previously cited for a repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 1364060, citation number 2, item number 1, and was affirmed as a final order on 2/11/2019, with respect to a workplace located at 3653 Market St., Fairlawn, OH, 44333.  Chanell Roofing & Home Improvement, LLC was previously cited for a serious violation of this occupational safety and health standard, which was contained in OSHA inspection number 1265206, citation number 1, item 1, and was affirmed as a final order on 11/17/2017, with respect to a workplace located at 9410 Rockys Ct., Broadview Heights, OH, 44147.
