29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about December 4, 2018, employees were exposed to a fall of approximately 22 feet while installing roofing materials on a commercial building located at 3653 West Market, Fairlawn, Ohio 44333. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about December 4, 2018, employees were exposed to a fall of approximately 9 feet while installing roofing materials on a commercial building located at 3653 West Market, Fairlawn, Ohio 44333. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area. Chanell Roofing & Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1265206, citation number 2, item number 1 and was affirmed as a final order on 11/17/2017, with respect to a workplace located at 9410 Rocky's Ct, Broadview Heights, Ohio 44147.
