29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  1001 Sullivan Rd., Aurora, IL 60506, North, Recycling Department, 8-inch Extruder: on or about October 12, 2017, the employer did not provide hazardous energy control (lockout/tagout) authorized employee training to extruder operators (affected employees) that engaged in servicing tasks to unjam clogged screw auger(s) without de-energizing. Operators had their fingers and hands exposed to moving/rotating parts that created caught-in hazards when using hand tools to service equipment with safeguards removed.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (a) prepare for shut down by recognizing energy hazards and its controls, (b) turn off and shut down the machines, (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (d) affix lockout or tagout devices to each energy isolating device, (e) render safe all potentially hazardous stored or residual energy, (f) verify that isolation and de-energization of the machine or equipment have been accomplished.  1001 Sullivan Rd., Aurora, IL 60506, North, Recycling Department, 8-inch Extruder: on or about October 12, 2017, extruder operators performing servicing tasks to unjam clogged screw auger were not required to apply energy controls. Operators had their fingers and hands exposed to danger zone rotating parts caught-in hazards when using hand tools to service equipment with removed safeguards because hazardous energy controls (lockout) were not implemented.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
