29 CFR 1926.416(a)(1):  Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a)  On or about February 16, 2016, employees were exposed to electrical shock while working in close proximity to a 120/240 volt overhead power line.     No abatement certification or documentation is required for this item.

29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit.  The employer did not post and maintain proper warming signs where such a circuit exists.  The employer did not advise employees of the locations of such lines, the hazards involved, and the protective measures to be taken:  a)  On or about February 12-15, 2018, the employer did not determine whether electric power circuits could be contacted by employees working at the site.  b)  On or about February 12-15, 2018, the employer did not advise employees of the locations of such lines, the hazards involved, and the protective measures to be taken.  c)  On or about February 12-15, 2018, the employer did not post and maintain warning signs where energized electric power circuits existed.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  On February 21, 2018, employees performing carpentry work were exposed to fall hazards of approximately 14.4 feet.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
