29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On 5/4/2018, employees of Roofs by Antonio LLC were working on a roof of a residential building located at 2108 Lee Ridge Drive, O'Fallon MO.  Eye protection was not used by employees involved with using pneumatic nail guns during roofing operations.  Roofs by Antonio, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a), which was contained in OSHA inspection number 1290720, citation number 1, item number 1 and was affirmed as a final order on 2/8/2018, with respect to a workplace located at 7933 Croce Circle, Denver, Colorado.   Roofs by Antonio, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a), which was contained in OSHA inspection number 975023, citation number 1, item number 1 and was affirmed as a final order on 7/29/2014, with respect to a workplace located at 7672 Montarbor Drive, Colorado Springs, Colorado

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On 5/4/2018, employees of Roofs by Antonio were working on the roof of a residential building located at 2108 Lee Ridge Drive, O'Fallon MO.  Employees were performing roofing work 10 feet above a lower level.  Exposed employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.  Roofs by Antonio, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1046922, citation number 2, item number 1 and was affirmed as a final order on 10/15/2015, with respect to a workplace located at 200 East Home Avenue, Palatine, IL. 29 CFR 1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 5/4/2018, employees of Roofs by Antonio LLC were working on a roof of a residential building located at 2108 Lee Ridge Drive, O'Fallon MO. The ladder the workers were using to access the roof was not extended three feet beyond the landing or edge of the roof.
