The employer did not ensure an extension ladder used for accessing and egressing the roofing work was properly positioned. The employer allowed employees to climb up a foldable extension ladder while carrying bundles of shingles in their hand. The workers climbing the ladder were at times not grasping the ladder with either hand exposing themselves to a 14-foot fall hazard.
