29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  a) 2609 Rising Sun Road, Schnecksville, PA, Worksite - on or about July 5, 2018, multiple plastic fuel cans containing more than five gallons of gasoline were in use as the ready fuel supply for worksite equipment, but a fire extinguisher of the necessary rating was not available.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 2609 Rising Sun Road, Schnecksville, PA, Worksite - on or about July 5, 2018, employees were using an extension ladder to access a residential roof, approximately nine feet above grade, that was not extended at least three feet above the upper landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) 2609 Rising Sun Road, Schnecksville, PA, Worksite, Roof - The employer does not ensure that appropriate fall protection equipment or methods are in use. A specific instance of this continuing violation was most recently observed on or about July 5, 2018 when employees were performing residential roofing activities while working without fall protection at the unprotected edges of a steeply pitched roof, approximately nine to 30 feet above a lower level. The employer's ongoing obligation is to ensure that fall protection equipment and/or methods are in use when employees are exposed to fall hazards.
