29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Construction site, Building group 8 Unit 47 - On or about July 18, 2018, employees were exposed to a fall hazard greater than 6 feet while walking on the top plate of the second floor setting trusses did not utilize a fall protection plan to mitigate the hazard or conventional fall protection. b) Construction site, Building group 8 Unit 47 second floor - On or about July 18, 2018, employees walked/worked in near proximity to an unguarded opening for future stairs, exposing them to a fall hazard greater than 6 feet. c) Construction site, Building group 8 Unit 47 second floor - On or about July 18, 2018, employees walked/worked in near proximity to unguarded window openings with a sill height of 24 inches, exposing them to a fall hazard greater than 6 feet.

29 CFR 1926.1428(a): The employer of the signal person did not ensure that each signal person met the Qualifications Requirements (paragraph (c) of this section) prior to giving any signals: a) Construction site, Building group 8 - On or about July 18, 2018, Employees who had not been trained to meet the required qualifications were signaling and directing a crane hoisting trusses and were exposed to struck by hazards.
