This most recently occurred on August 1, 2018 on the jobsite located on Lot 33 of the Granite Falls North Subdivision on Cove Hollow Cir, Papillion, NE 68046. Two employees were observed using hand-held pneumatic nail guns to install roofing material without any form of eye or face protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This most recently occurred on August 1, 2018 on the jobsite located on Lot 33 of the Granite Falls North Subdivision on Cove Hollow Cir, Papillion, NE 68046. The owner and four employees are using an aluminum extension ladder to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about twenty-one feet above the ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on August 1, 2018 on the jobsite located on Lot 33 of the Granite Falls North Subdivision on Cove Hollow Cir, Papillion, NE 68046. The company owner and four employees were re-roofing a two-story residential house and not utilizing any form of fall protection. The house has a five in twelve steep sloped pitch and ground to eve height is approximately twenty-one feet. Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1007101, citation number 2, item number 1, and was affirmed as a final order on February 12, 2015 with respect to a workplace located at 2469 182nd Cir, Omaha, NE 68130. Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1127759, citation number 1, item number 1, and was affirmed as a final order on April 3, 2017 with respect to a workplace located at 6611 S. 200th Ave, Omaha, NE 68135.
