This was most recently documented on or about May 23, 2022 at a residential construction jobsite located at 4513 S 215 Ave, Elkhorn Nebraska. The employer did not ensure that debris was not collecting on around the base of the ladders being utilized at the jobsite.

This was most recently documented on or about May 23, 2022 at a residential construction jobsite located at 4513 S 215 Ave, Elkhorn Nebraska. The employer did not ensure that a damaged aluminum extension ladder being used as means of access and egress to the upper working areas by the employees was inspected for visible defects.
