29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  This was most recently observed on April 19, 2019 at 18314 Grant St., Elkhorn, Nebraska, 68022. Four employees were working on the roof of a residential structure without utilizing fall protection. Four employees were installing shingles on a newly constructed two-story residential home and not utilizing any form of fall protection.  The ground to eave height is approximately twelve feet.        Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1127759, citation number 1, item number 1, and was affirmed as a final order on April 3, 2017 with respect to a workplace located at 6611 S. 200th Ave, Omaha, NE 68135.      Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1163496, citation number 1, item number 1, and was affirmed as a final order on April 12, 2017 with respect to a workplace located at 2202 176th St, Elkhorn, NE 68122.    Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1335198, citation number 2, item number 1, and was affirmed as a final order on January 16, 2019 with respect to a workplace located at Cove Hollow Cir, Lot 33, Papillion, NE 68046.    Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1349124, citation number 1, item number 1, and was affirmed as a final order on January 16, 2019 with respect to a workplace located at 19256 Ames Cir, Omaha, NE 68116.
