29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  The employer is failing protect employees from projectile hazards.  This was most recently documented on or about  May 3rd, 2023, at 18683 Oakmont Drive, Omaha, Nebraska 68136.  Employees were observed using a pneumatic nail gun without eye protection.     Francisco Esquivel Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1629149, citation number 2, item number 1 and was affirmed as a final order on February 7th, 2023 with respect to a workplace located at 12612 South 74t Street, Papillion, Nebraska 68046.  Francisco Esquivel Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1598343, citation number 3, item number 1 and was affirmed as a final order on August 19, 2022, with respect to a workplace located at 4513 S 215 Ave, Elkhorn Nebraska. The employer is failing to protect employees from fall hazards.  This was most recently documented on or about May 3rd, 2023 at  18683 Oakmont Drive, Omaha, Nebraska 68136.  Multiple employees were observed conducting roofing operations at heights greater than 6 feet without fall protection.    Francisco Esquivel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1629149, citation number 1, item number 1 and was affirmed as a final order on February 7th, 2023 with respect to a workplace located at 12612 S 74th St. Papillion Nebraska. The employer is failing to protect employees from fall hazards.  This was most recently documented on or about May 3rd, 2023, at 18683 Oakmont Drive, Omaha, Nebraska 68136.  Two employees were observed using an extension ladder that did not extend 3 feet above the upped landing surface.
