29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Location: Endicott Woods - 70 Endicott St., Norwood, MA 02062 On or about August 2, 2018, the roof anchors, installed and used by employees engaged in roofing activities, were not capable of supporting at least 5,000 pounds (22.2 kN), where the anchors used were not part of a complete personal fall arrest system that maintains a safety factor of at least two and were not designed, installed, and used under the supervision of a qualified person.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: Location: Endicott Woods - 70 Endicott St., Norwood, MA 02062 On or about August 2, 2018 the employer did not ensure that employees were protected from falls of more than 6 feet and exceeding 27 feet, by allowing the employees to gain access to a upper level using the shingle platform hoist as a ladder.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b) Location: Endicott Woods - 70 Endicott St., Norwood, MA 02062 On or about August 2, 2018 employees were exposed to fall hazards of more than 27 feet to the ground below from the roof, where adequate fall protection was not used, while conducting roofing operations. JMT Carpentry & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1317715, citation number 1, item number 2 and was affirmed as a final order on 6/6/18, with respect to a workplace located at 135 Plymouth Street Abington, MA 02351
