29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   (a) Worksite:  On or about 8/29/2018, eye and face protective equipment was not used when employees were operating pneumatic nail guns.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Worksite:  On or about 8/29/2018, employees were not protected from falling while working on a roof approximately thirty-one feet above a lower level.  JMT Carpentry & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1317715, citation number 001, item number 02 and was affirmed as a final order on 6/6/2018, with respect to a workplace located at 135 Plymouth Street, Abington MA, 02351.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   (a) Worksite:  On or about 8/29/2018, employees were not protected from falling while using a ladder that did not extend at least three feet above the upper landing surface.  JMT Carpentry & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1317715, citation number 001, item number 03a and was affirmed as a final order on 6/6/2018, with respect to a workplace located at 135 Plymouth Street, Abington MA, 02351.
